To all 14th Pan American Maccabi Games Masters Team Members:

Congratulations again on your appointment to Team USA! We are looking forward to you joining us in Mexico City this summer.

This form is for you to request any additional hotel options at the Four Seasons (our Masters team hotel), including companions, family members, additional rooms, single rooms, and additional nights.

Included in your $6,000 finanical commitment is a double-occupancy room shared with another USA Masters Team Member. If you wish to stay with this arrangement, please ignore this form and no further action is required.

Nights included as part of the Games are July 5-14. Check-in is on July 5 (afternoon) and check-out is on July 15 (morning). Additional nights would be for outside this window. Transportation and meals are only provided July 5-15. 

If you will be registering any additional family members, they will receive the following:

• Visitor Pass (pass includes Opening Ceremony & compeition tickets)
• Lodging at Four Seasons
• Breakfast at Four Seasons, lunch & dinner at the CDI
• Ground transportation
• Health insurance
• Select social & cultural events

Flights are not included for any Masters athlete or family members registered.

These options are for Masters Team Members only.

This form and payment is due no later than Friday, March 15 at 3:00pm EST. No refunds will be processed after this time.
